PACE (Programs of All-Inclusive Care for the Elderly) is an innovative model of care that provides a full range of medical and long-term services and supports intended to allow nursing home eligible older adults to maintain their independence and age gracefully in the community. The PACE model emphasizes comprehensive, coordinated, and preventative care in a way that treats the whole person – addressing physical, emotional, and psychological needs.

PACE is poised for growth in a post-Covid world where the benefits of aging-in-place have been amplified and technology and other changes provide enhanced opportunities for supportive models of care. One goal of this session is to increase the awareness and understanding of PACE among innovators in aging.

Bridging the Gap - Day 2

This presentation is part of Bridging the Gap - Day 2 which features sessions from Aging2.0 chapters across the world including Maine, Halifax, Portugal, Philadelphia, Mexico, New York and our own Kansas City chapter.

Topics that will be discussed include unique Covid-19 adaptations and solutions that have been succeeding, how the traditional models of elder care are being turned upside down, programs and strategies being put in place to succeed in the new world we face, and much more. Attendees will have the opportunity to participate through live Q&A’s and an extended “Ask Me Anything” with LHCC CEO Tammy York Day. Additionally, we’ve invited some special guests to join and share their incredible work in the field of aging, including a keynote from the one and only Chip Conley, former Airbnb exec and founder of the Modern Elder Academy.
