For older adults, social determinants significantly impact their health, well-being, and their ability to live independently.
Join us for an interactive discussion with industry leaders from the Institute on Aging and Uber Health to learn about their initiatives to support Social Determinants of Health (SDOH) for older adults.
Social Determinants of Health are social, economic, and environmental conditions that help older adults maintain their health and well-being. These factors include things such as nutrition, social connectedness, reliable transportation and access to health care services.
Presented by Aging2.0 Palo Alto and San Francisco Chapters.
