The Next Frontier of Value-Based Care: Pre/Post Acute

Older adults represent the largest and fastest growing consumer of healthcare services around the world. In the US, the healthcare system is being redesigned to better align incentives, decrease costs and improve health outcomes. The changing incentive structures are creating new priorities and payment mechanisms within the healthcare system − in particular a focus on post-acute care, care management and providing quality care in the least expensive setting possible. New Medicare codes and bundled payments are also creating new opportunities (and funding streams!) around chronic care management, care transitions, advanced care planning, telemedicine, non-medical home care and more.

These strong regulatory tailwinds not only create new demand for outcomes data and tech-enabled care models, but also creates new potential payors who have an increasingly vested interest in delivering high quality, cost-effective care. Hear from Medicare Advantage plans and other risk takers about how they are partnering with post-acute providers and leveraging technology to capitalize on these trends and opportunities.

Japan is a “super-aging” society with the largest percentage of older adults in the world. Aging2.0 Leaders Circle partner, SOMPO, is one of the largest insurers and the largest senior housing provider in Japan. They’re leading the effort to innovate senior care to support Japan’s aging population. To that end, SOMPO will be hosting a pitch event for high-potential startups interested in the Japanese market. Grand Challenges in Aging

The Aging2.0 team will take a look back on the progress made across the Grand Challenges during 2018, with a focus on Caregiving and Brain Health. Then we’ll dive in with a powerful narrative from a journalist turned full-time caregiver, who spent decades caring for her late husband with early-onset Alzheimer’s and now her own mother. The session will wrap with a showcase of some of the best-of-the-best startups who pitched over the summer at the Aging2.0 Grand Challenge Forum events on Caregiving and Brain Health
